Family Preservation Case Worker

Summary:

The Family Preservation Case Worker plays a crucial role in supporting families in crisis within the Mount Druitt community. This position focuses on providing intensive case management and support services to ensure the safety and well-being of children while promoting family unity. The role is essential in helping Aboriginal families navigate challenges and access necessary resources, ultimately contributing to the overall health of the community.

Responsibilities:

• Conduct comprehensive assessments of families to identify needs and strengths.
• Develop and implement individualised case plans in collaboration with families.
• Provide ongoing support and guidance to families, ensuring they have access to relevant services.
• Facilitate family meetings and coordinate with external agencies to enhance service delivery.
• Monitor and evaluate the progress of families against established goals.
• Maintain accurate and up-to-date case records and documentation.
• Participate in training and professional development opportunities to enhance skills and knowledge.
• Advocate for Aboriginal families within the community and with service providers.

Qualifications:

• A relevant qualification in social work, psychology, or a related field.
• Experience in case management or working with Aboriginal families in crisis.
• Strong understanding of child protection legislation and family support services.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with diverse families and community organisations.
• Strong organisational skills and attention to detail.
• A valid driver’s licence.
