We're looking for experienced Clinical Assessment Delegates to join our aged care assessment team. If you're a qualified clinician with current My Aged Care assessment and delegation credentials, this is an opportunity to make a genuine impact while enjoying flexibility, autonomy, and strong organisational support.

What You'll Do

As a Clinical Assessment Delegate, you'll use your clinical expertise and delegated authority to:

• Conduct and finalise aged care assessments under the Single Assessment System
• Determine eligibility for aged care services and supports
• Develop support plans and issue decision notices
• Review and update client support plans to ensure ongoing needs are met
• Maintain accurate records within My Aged Care and internal systems
• Work collaboratively with clients, carers, service providers, and key stakeholders
• Ensure compliance with legislative, quality, and clinical governance requirements

You'll manage your own caseload and play a vital role in helping older Australians access timely and appropriate care.

About You

To succeed in this role, you'll have:

• At least 12 months' experience conducting Aged Care Needs Assessments
• Previous experience as a My Aged Care Clinical Delegate
• Strong knowledge of My Aged Care assessment frameworks, processes, and eligibility pathways
• Sound clinical reasoning and decision-making skills
• Excellent written communication and documentation skills
• The ability to work autonomously and manage competing priorities
• Confidence using digital systems and technology

Essential Qualifications

• Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker qualification
• Current AHPRA registration, or eligibility for membership with the Australian Association of Social Workers (AASW)
• Current Clinical Delegate Certification
• Current Clinical Aged Care Needs Assessor Certification
• Completion of required My Aged Care Learning modules and ongoing refresher training requirements
• Current influenza vaccination (or willingness to obtain as required)
