The Opportunity:
The Renal Health Program, within the Wellbeing and Preventable Chronic Disease Division, works in partnership with government and non-government health services across the Northern Territory and nationally to improve the prevention and management of chronic and kidney disease.
We are seeking an experienced and motivated Clinical Nurse Specialist to support the widespread implementation and uptake of Territory Kidney Care (TKC).TKC is a clinical decision support tool designed to support healthcare professionals in delivering evidence-based care for people with chronic and kidney disease. In this role, you will play a key part in supporting clinicians and health services to embed TKC into everyday practice and maximise its benefits for patients and the health system.

You will be an experienced nursing professional with strong clinical knowledge, excellent communication skills and a passion for improving health outcomes through innovation and service improvement.
