Ready to take the next step in your disability services career?
Community Care Foundation is looking for an experienced Participant Services Facilitator to join our Participant Services team and help lead the delivery of high-quality, person-centred supports across the Fraser Coast and Wide Bay.
This is a hands-on coordination role for someone who understands that great disability services are about more than rosters and paperwork — they are about people, relationships, quality and outcomes.
Working closely with participants, their stakeholders and our Lifestyle Support Workers, you will help ensure our services are safe, compliant, well-coordinated and focused on achieving meaningful outcomes for the people we support.

The Role
As a Participant Services Facilitator, you will play an important role in the day-to-day coordination and quality oversight of allocated participant services and SIL operations.
Your responsibilities will include:
