ndis support coordinator

Key Duties

• Support NDIS participants to understand and effectively utilise their NDIS plans.

• Connect participants with suitable NDIS, community and mainstream services.

• Coordinate supports and liaise with families, providers, allied health professionals and other stakeholders.

• Promote participant choice, control, independence and capacity building.

• Monitor plan implementation, funding utilisation and progress towards participant goals.

• Identify service gaps, risks and changes in support needs and coordinate appropriate responses.

• Assist with NDIS plan reassessments/reviews, reports and supporting documentation.

• Maintain accurate case notes, participant records and required documentation.

• Participate in stakeholder meetings and case conferences as required.

• Work in accordance with the NDIS Code of Conduct, Practice Standards and Align Care policies and procedures.
