Team members are encouraged to uphold values of respect, dignity, and empowerment in every interaction with the people they support.Role Description The Support Coordinator is a full-time, on-site role based in Canberra, responsible for helping people with disability navigate and coordinate their supports. Day-to-day tasks include working with individuals to understand their needs and goals, developing support plans, and connecting them with appropriate services and resources. The role involves coordinating case management activities, monitoring progress, and adjusting supports as circumstances change. The Support Coordinator will liaise with service providers, government agencies, and community organizations, maintain accurate documentation, and ensure that services are delivered in line with relevant standards and funding arrangements. This position requires consistent collaboration with participants, families, and colleagues to promote positive outcomes and a high quality of service.Qualifications Candidates should possess strong Interpersonal Skills and Communication abilities to build trust, collaborate effectively, and explain complex information clearly.Candidates should possess Case Management skills to assess needs, develop support plans, coordinate services, and monitor outcomes.Candidates should possess Customer Service skills to respond promptly to inquiries, manage expectations, and provide a supportive experience for participants and their networks.Candidates should possess Technical Support skills to use client management systems, maintain accurate records, and navigate relevant online portals or platforms.Relevant experience in disability, community services, or aged care, with knowledge of local support systems and funding frameworks (such as NDIS) is highly beneficial.Ability to work on-site in Canberra, manage competing priorities, and maintain confidentiality and ethical practice in all aspects of the role.Formal qualifications in social work, community services, psychology, or a related field, or equivalent experience in a similar role, are desirable.
