About the Role

An opportunity exists for an experienced and motivated Perioperative Liaison Nurse to join Monash Health's Elective Surgery Services (ESS) team on a Full Time Ongoing basis. This is a Monday – Friday role, typically 8:00am – 4:30pm, however is rostered according to operational requirements.

This role is central to delivering a contemporary elective surgery model of care, acting as the single point of contact for patients throughout their surgical journey. Working closely with patients, families and multidisciplinary teams, you will coordinate and optimise patients for elective surgery, ensuring they are clinically prepared, appropriately scheduled and supported from referral through to the day of surgery.

You will play a key role in improving the patient experience, reducing day-of-surgery cancellations, managing elective surgery waitlists in accordance with the Elective Surgery Access Policy (ESAP), and supporting the efficient utilisation of theatre capacity across Monash Health. The role also involves travel across Monash Health campuses based on service requirements.

In This Role You Will

• Assess patient readiness for surgery and coordinate optimisation where required.
• Develop individualised care pathways for patients with complex health needs.
• Collaborate with surgeons, anaesthetists, allied health and community providers to facilitate timely and safe surgical care.
• Provide education and support to patients and families before surgery.
• Manage elective surgery waitlists and contribute to reducing delays and day-of-surgery cancellations.
• Drive quality improvement initiatives that enhance patient experience and support ESAP compliance.

About You

You are an experienced perioperative or surgical nurse who enjoys coordinating care, building relationships and improving patient outcomes. You thrive in a collaborative environment and are passionate about delivering person-centred care while navigating complex patient journeys.

To be successful in this role, you will bring:

• Current registration as a Registered Nurse with AHPRA, essential
• A minimum of five years' surgical nursing experience, essential
• Demonstrated experience working in a leadership or care coordination capacity, essential
• Strong clinical assessment skills and the ability to manage complex patient pathways.
• Highly developed written, verbal and interpersonal communication skills.
• Excellent organisational skills with the ability to work independently and manage competing priorities.
• A collaborative, adaptable and patient-focused approach.
• Current Victorian Driver's Licence, essential
• A master’s degree in nursing (or working towards) is desirable.
