About the Role

As the Pathways Housing & Support Coordinator, you will provide culturally safe person-centred, strengths-based case management, outreach support and tenancy support to individuals and families accessing the Pathways program.

Working closely with other specialist agencies, you will support clients to access housing, healthcare, legal and migration services, education, employment pathways and essential supports while assisting them to navigate the challenges associated with visa uncertainty.

You will also contribute to the ongoing growth, innovation and quality improvement of YWCA Canberra’s Housing Support Unit.

Key Responsibilities

Case Management and Support

• Undertake comprehensive assessments and develop individualised support plans.
• Provide proactive outreach and case management support to individuals, children, young people and families.
• Support access to housing, healthcare, migration and legal services, education and employment opportunities.
• Provide advocacy and coordinated referrals to internal and external support services.
• Facilitate and participate in case conferences and collaborative service planning.
• Maintain accurate case notes, client records and reporting requirements.

Family and Child Support

• Deliver child-centred and family-focused support.
• Undertake child and family assessments and develop tailored case management plans.
• Strengthen family relationships and build parental capacity and resilience.
• Support positive outcomes for children and young people experiencing housing instability.

Housing and Tenancy Support

• Support clients in transitional and headleased accommodation.
• Assist with tenancy sustainment and housing transitions.
• Work collaboratively with tenancy staff to resolve tenancy issues and promote successful housing outcomes.

Collaboration and Service Development

• Work closely with other specialist services and stakeholders.
• Build and maintain strong sector partnerships and referral pathways.
• Participate in continuous improvement, reflective practice and professional development activities.
• Contribute to innovative service responses that improve client outcomes.

About You

You are an empathetic, resilient and highly organised practitioner who is passionate about social justice and supporting vulnerable communities.

Essential

• A minimum Diploma in Community Services or tertiary qualifications in Social Work, Psychology, Human Services or a related discipline.
• Experience working with people experiencing disadvantage, homelessness or complex needs.
• Demonstrated experience in case management, advocacy and support coordination.
• Understanding of trauma-informed, strengths-based and culturally responsive practice.
• Strong written and verbal communication skills.
• Ability to manage competing priorities and work both independently and collaboratively.
• Current Working with Vulnerable People (WWVP) Registration.
• Current driver’s licence and access to a comprehensively insured vehicle.
• Full Australian working rights.

Desirable

• Experience working with people seeking asylum or experiencing visa uncertainty.
• Experience supporting families, children and young people.
• Knowledge of homelessness and housing systems.
• Qualifications or training in child and family development.

WWVP Requirements
All YWCA Canberra employees are required to hold and maintain a valid Working with Vulnerable People (WWVP) registration for the duration of their employment. We operate under a strict ‘No card, no start’ policy, meaning this clearance must be obtained prior to commencing in any role.
