We are looking for a Care Partner who takes ownership of their clients, follows through, and wants to be part of a professional team.
The Role
You will manage your own portfolio of clients and be their trusted point of contact. You will coordinate services, maintain care plans, budgets and ensure clients receive responsive support every day.
The role exists to ensure clients receive personalised, timely, and high quality support that enhances independence and quality of life.
What You Will Bring
· Experience in aged care, home care, or community services
· Strong communication and relationship management skills
· Ability to work independently and stay organised
· A proactive, reliable approach to your work
