Who we are
We are a not-for-profit provider of personalised in-home aged care services, supporting older people to live independently with dignity. Our team delivers practical, culturally appropriate care, with a strong focus on trust, respect, and consistency. As the sector transitions to Support at Home, we are expanding and seeking an experienced Care Partner to support clients through this change.
The role
This is a care management and coordination role focused on delivering Support at Home services. You will work closely with clients, families, and providers to plan, coordinate, and review services in line with changing needs.
You will need a solid understanding of Support at Home, including program structure, funding, and operational requirements. This role combines care planning, coordination, administration, and client engagement, with time split between office and client visits.
While non-clinical, you will coordinate access to allied health, nursing, equipment, and home modifications where required.
Key responsibilities
• Conduct Support at Home assessments and reviews in clients’ homes
• Manage intake, onboarding, and service agreements
• Develop and maintain care plans, budgets, and documentation
• Coordinate services with internal teams and external providers
• Arrange allied health, nursing supports, and AT-HM services
• Monitor budgets and funding utilisation
• Adjust services in response to changing client needs
• Work with rostering teams and care staff to ensure service delivery
• Manage feedback and complaints in line with Quality Standards
• Maintain accurate records and support continuous improvement
About you
• Experience in Support at Home and/or Home Care Package coordination
• Strong understanding of aged care programs and service delivery
• Confident in care planning, coordination, and documentation
• Experience managing budgets and funding
• Strong communication and relationship-building skills
• Able to work independently and as part of a team
• Proficient in client systems and Microsoft Office
Requirements
• Driver’s licence and reliable vehicle
• National Police Check (or willingness to obtain)
• Full Australian work rights
Desirable
• Experience coordinating allied health or third-party providers
• Knowledge of Aged Care Quality Standards
• Experience working through aged care reform
