The Frankston Adult Community Mental Health Team (FACMHT), based at Tarnbuk Centre in Frankston, provides specialist assessment, treatment, and case management services to adults aged 16 to 65 years experiencing significant mental health conditions, as well as support to their carers and families. Consumers accessing the service present with a wide range of mental health diagnoses and often experience co-occurring conditions, including substance use disorders and complex psychosocial issues.
The Case Management team delivers assertive, recovery-oriented community treatment and works collaboratively within a multidisciplinary framework to ensure safe, effective, and coordinated care.
The Community Mental Health Clinician, as a member of the multidisciplinary team, provides timely and responsive mental health assessment, treatment, and support to individuals requiring urgent, short-term, or ongoing intensive community-based interventions. The clinician practices within their professional scope to deliver safe, evidence-based, person-centred care, supporting consumers to achieve optimal health and recovery outcomes.
