Where you'll be working
Join Western Sydney Local Health District's dedicated Social Work team, providing high-quality care across Blacktown and Mount Druitt Hospitals. You'll become part of a collaborative multidisciplinary team committed to delivering person-centred care that improves health outcomes for patients, families and carers. Working across Rehabilitation, Aged Care and Stroke Services, you'll have the opportunity to make a meaningful impact while developing your clinical expertise in a supportive and inclusive environment.
Work across inpatient and outpatient Rehabilitation, Aged Care and Stroke Services
Collaborate with experienced medical, nursing and allied health professionals
Receive regular clinical supervision, mentoring and professional development
Contribute to quality improvement, education and evidence-based practice

What you'll be doing
As a Social Worker Level 1/2, you will provide comprehensive social work services to patients, families and carers experiencing a broad range of psychosocial and complex care needs. You'll apply professional judgement to deliver high-quality interventions while working collaboratively with multidisciplinary teams to support safe, patient-centred care.
Deliver comprehensive social work assessment, intervention and discharge planning
Support patients and families through complex issues including safeguarding, domestic violence, elder abuse, guardianship and grief
Collaborate with multidisciplinary teams to achieve positive patient outcomes
Participate in clinical supervision, professional development, quality improvement and service development activities
Contribute to education, student supervision and evidence-based practice initiatives

Essential Criteria
Qualification : Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW)
Selection Criteria
Demonstrated clinical experience or understanding of the needs of clients accessing the health care system.
Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client group.
Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills.
Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
Knowledge of quality assurance and / or clinical practice improvement activities.
Ability to participate in the After Hours Crisis On Call Social Work Service.
