CNHC’s Service Coordinator is the primary point of contact for all our clients and is responsible for assessing, scheduling, and monitoring services to clients and providing individualised care to enable our clients maintain independence and connection to the community.
All responsibilities must be undertaken in line with the Policies and Procedures of Complete Nursing & Home Care Pty Ltd and under the NDIS guidelines. The Service Coordinator embodies the company values of Respect, Integrity, Quality and Commitment.
Skills Required
Strong verbal and written communication.
Proficient computer and administrative skills.
Ability to maintain confidentiality and privacy.
Strong organisational, time management, and multitasking abilities.
Attention to detail and accuracy.
Problem-solving skills and ability to work under pressure.
With current Victorian Driver's Licence and confident to drive
Desirable Qualifications
Certificate in Disability/Aged Care or Community work is desirable
Relevant qualification in the disability sector
Prior rostering experience
Extensive knowledge of NDIS and Practice Standards
Experience working in the welfare/community services sector, particularly in disability
Experience or knowledge of current My Aged Care policies
Ongoing Duties and Responsibilities
To work with individuals and their families to schedule care and support including personal care, clinical support and engaging with customers and communities.
To manage consumer directed care queries flexible and tailored coordination to meet specific customers' needs and incorporate shared care planning.
Ability to work with company’s CRM rostering system. Maintain participant and staff information within scheduling and CRM systems.
To use strengths-based approach together with a strong collaboration approach to assessing and matching carers and services in the areas of need where the customer and their immediate supports cannot provide.
To ensure workers allocated to shifts possess required qualifications, screening checks, and competencies.
To work closely with Senior Administrator Officer for service agreements.
Ability to support compliance with NDIS Practice Standards and organisational policies.
To ensure the services are delivered in line with the company's policies and procedures.
To lead immediate response to Incident Reports, follow up, and management. Receive, acknowledge, accurately document and escalate complaints, concern safeguarding issues, incidents or service quality concerns to the manager within required timeframes.
To demonstrate awareness of diversity and be respectful to all stakeholders of the enterprise.
To demonstrate a commitment to ongoing professional development.
To demonstrate effective communication skills with customers, staff, and allied health professionals.
To understand the risks associated with work and ways to mitigate these.
To assist with payroll and timesheet reconciliation processes.
Other related tasks consistent with your skills and training.
