Disability Support Worker - Centre Based Programs

About the role

We are seeking compassionate, reliable and motivated Disability & Aged Care Support Workers to join our team and provide high-quality, person-centered support to clients within the community. This is a hands-on role where you will support individuals to maintain their independence, participate in their community and achieve their personal goals. No two days are the same, with support tailored to each client's individual needs, preferences and support plan.

EDGE Community Services provides the following services across our sites:

• Centre-Based Day Programs (Hoppers Crossing & Geelong)

• Short-Term Respite/ Overnight Stays

• Aged Care Community Access

• Transport Accident Commission (TAC) participant supports

• 1:1 Support Services

Key responsibilities

• Providing active support to clients and encouraging independence and choice.

• Supporting clients with personal care and activities of daily living, where required.

• Providing safe and appropriate community access and facilitation.

• Supporting clients to attend appointments, shopping, social activities, recreational activities and community events.

• Assisting clients to build confidence and maintain connections within their community.

• Supporting clients with meal preparation, household tasks and other day-to-day activities.

• Following and implementing Behavior Support Plans (BSPs) and individual support strategies.

• Supporting clients who may display behaviors of concern using positive, person-centered approaches.

• Assisting clients with communication and ensuring their individual communication needs are respected.

• Monitoring and reporting changes in a client's wellbeing, behavior or support needs.

• Reliable, responsible and professional.

• Confident supporting people both independently and in the community.

• Comfortable providing personal care where required.

• Able to follow individual support plans and Behavior Support Plans.

• Confident supporting clients with different physical, social and behavioral needs.

• Respectful of each person's dignity, privacy, choices and independence.

• A strong communicator with a positive and flexible attitude.

• Comfortable driving clients or assisting with transport where required (may include driving a 12-seater)

• Previous experience in disability or aged care is highly regarded. Relevant qualifications such as a Certificate III/IV in Individual Support, Disability or Aged Care are desirable.
