About the Role

We are seeking two Support Coordinators to join our Housing First Support Service, a consortium led by Ruah Community Services in partnership with St Patrick's Community Support Centre.

This program supports people aged 16 years and over who are experiencing homelessness to secure sustainable housing and maintain their tenancy through individualised, person-centred support.

As a Support Coordinator, you will provide case management and psychosocial support to assist clients in improving their quality of life, health, independence, and housing outcomes. Working from a strengths-based and recovery-oriented approach, you will support clients to identify and achieve their goals, coordinate services, advocate on their behalf, and build pathways toward long-term housing stability.

This is a full-time position working 38 hours per week, Monday to Friday, on a maximum-term contract until 30 June 2027.

What You'll Be Doing

• Delivering person-centred case management and support interventions
• Supporting clients to secure, establish and sustain long-term housing
• Developing and monitoring individual support plans and goals
• Working collaboratively with housing providers, community services and government agencies
• Advocating for clients to access health, mental health, AOD, financial, legal and social supports
• Maintaining accurate client records, reports and case notes
• Managing complex and competing priorities within a fast-paced service environment
• Contributing to a positive and collaborative team culture

What We're Looking For

The successful applicants will be required to meet the following criteria:

• Relevant qualifications in a related discipline at degree, diploma, or certificate level; less formal qualifications with relevant work or lived experience; or demonstrated experience performing at this level
• Personal goal planning skills that contribute to the achievement of client goals in line with the Model of Care
• Skills in reflective practice and highly developed report writing using analytical reasoning
• Ability to support, mentor and work alongside supporting staff members
• Advanced computer skills with proficiency in Microsoft Office and client management systems
• Ability to engage and maintain professional relationships with clients and their broader support networks
• Demonstrated understanding of complex issues facing people experiencing homelessness, alcohol and other drug use, family and domestic violence, and mental health challenges
• Knowledge and understanding of diversity, gender and social inclusion, particularly Aboriginal culture
• A commitment to working within and contributing to the vision, mission, values and guiding principles of Ruah
• Knowledge and understanding of Housing First principles will be highly regarded

Highly Regarded

• Experience working within homelessness, housing, community services, mental health, or related sectors
• Experience working with people with multiple and complex needs
• Knowledge of trauma-informed and recovery-oriented practice
