Northern Health are seeking applications from motivated and suitably experienced Social Workers to join the Acute Social Work Team based at our Epping campus.

Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north.

Services are provided through our six main campuses:

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.

About the Role:

An exciting opportunity exists for an enthusiastic, dynamic and innovative Grade 2 Social Worker to join the Acute Social Work team at Northern Health Epping, on a permanent basis.  

This role requires a Grade 2 Social Worker to provide comprehensive psychosocial assessments, interventions and discharge planning to patients and their families on the Women's and Children's inpatient wards at our Epping site.  

To be successful in this role you will have:

• Highly developed interpersonal, practical negotiation, conflict resolution and communication skills. 
• Excellent organization skills, yet be flexible, to enable you to meet tight deadlines. 
• Excellent and concise documentation skills.  
• A demonstrated capacity to work as a team player both within the multi-disciplinary team, the Acute Social Work team, sepecially with Ante-natal Social Workers and be an excellent communicator who is comfortable engaging productively with staff at all levels of the health service. 
• Significant experience in undertaking comprehensive psychosocial assessments, intervention, discharge planning of infants, children, adolscents and adult patients within a hospital inpatient setting.
• Demonstrated knowledge of, and proven ability to access appropriate supports (eg. Family Violence Services, Child & Family Services & other relevant agencies) to ensure appropriate discharge plans for, as well as a sound understanding of relevant legislation.
• Responsible for practicing at an Intermediate Practice Level in accordance with the Family Violence Multi-Agency Risk Assessment & Management (MARAM) Framework.
• A good working knowledge of and proven ability to access appropriate supports for and work with various sectors (eg. RACF, TCP programs, ACAS, housing services) to arrange appropriate discharge plans  
• A commitment to self-initiated continuing professional development.
