The Pharmacotherapy Nurse plays a pivotal role within the Peninsula Opioid Pharmacotherapy (POPI) clinic, delivering high-quality, person-centred care to individuals accessing opioid pharmacotherapy while linking them into other Alcohol and Other Drug (AOD) treatment services.
Working as part of a multidisciplinary team, the Pharmacotherapy Nurse acts as a key clinical liaison between Patients, General Practitioners (GPs), opioid pharmacotherapy prescribers, community pharmacies and internal and external service providers to ensure coordinated, integrated and evidence-based care.
The role provides comprehensive nursing assessment and support, including obtaining substance use histories, contributing to multidisciplinary care planning, facilitating access to AOD treatment and harm reduction initiatives, delivering health education, providing psychosocial support and basic clinical interventions, and promoting continuity of care through effective cross-sector collaboration.
As part of a newly established Pharmacotherapy Clinic, the Pharmacotherapy Nurse contributes to the development of a welcoming, inclusive and trauma-informed service that promotes engagement, reduces barriers to treatment and supports improved health outcomes for people experiencing substance dependence.
The role is integral to fostering therapeutic relationships, advocating for patients and supporting recovery-oriented practice through collaborative, compassionate and culturally safe care. This is a permanent part time position working Monday to Friday. 30 hours per week.
