Programs Coordinator

At St John of God Murdoch Hospital our specialised palliative care is provided by health professionals who consider the physical, social, emotional and spiritual needs of our patients and their families and carers. We work with you to make the transition to home as smooth as possible by linking you with community support services.

We offer inpatient hospice care, specialist palliative care clinics and supportive therapies for people living with a life-limiting illness. Care may include support for pain and symptom relief, end-of-life care, advance care planning, and help to return home where appropriate. Patients and families can also access Footprints Day Centre, which offers emotional support, complementary therapies, wellness groups and craft workshops.

About the role

We are seeking a compassionate and motivated professional to coordinate and deliver meaningful programs and activities for individuals and groups within a palliative care setting. Working as part of a multidisciplinary team, you will contribute to the delivery of holistic, person-centred care that enhances the wellbeing and quality of life of patients and their families.

In this rewarding role, you will work closely with caregivers, clinicians and external service providers to ensure seamless care and support across the continuum. You will also foster strong connections with community nursing services, outpatient services, spiritual care providers and other community-based organisations to promote coordinated and comprehensive palliative care outcomes.

The role is offered on a fixed-term, 12-month part-time contract basis working 20 hours per week.

Key responsibilities

• Plan, coordinate and evaluate engaging therapeutic, recreational and creative programs for patients, carers and families.

• Collaborate with the interdisciplinary team to support holistic, person-centred palliative care and care planning.

• Provide individual support, follow-up care and complementary therapies within scope of practice to enhance patient wellbeing.

• Build and maintain strong relationships with community organisations, health providers and professional networks to support seamless care.

• Promote Support and Therapy services and contribute to community engagement, education and service awareness initiatives.

• Support and mentor volunteers, students and community placements in the delivery of palliative care programs and activities.

• Coordinate and contribute to special events, celebrations and awareness activities, including Palliative Care Week initiatives.

• Monitor patient and family wellbeing, communicating observations and concerns to the multidisciplinary team to support quality care outcomes.

Your skills and experience

To thrive in this role, you will have an understanding of the unique needs of palliative care patients and their Carers, together with excellent communication and interpersonal skills. You will be a compassionate team player who enjoys working collaboratively within a multidisciplinary environment, bringing experience in delivering complementary therapies and creative arts-based programs.

Previous experience in an inpatient healthcare setting and coordinating group programs will be highly regarded, as will an understanding of accreditation requirements and basic computer literacy skills.

• Salary: $77,239 to $80,741 per annum (pro-rata) plus 12% superannuation

• A fixed-term,12-month part-time contract, offering 20 hours per week
