Applied Coordinator Healthy Families Team / Nurse/Midwife Clinical Coordinator

Primary Objective: Provides a recognised reference point for the multi-disciplinary team in relation to assessment, planning, review, evaluation and coordination of service delivery in a defined practice area for groups and individuals with diverse requirements. Selection will be made at the AO7, P3 or N5 level depending on the experience/qualifications of the successful applicant.
