About Meditrax
Meditrax is a national clinical pharmacy and medication management provider working with residential aged care, home care and disability services across Australia. Our pharmacists are trusted by providers, general practitioners and families to make medicines safer for older people, and our work sits at the intersection of clinical practice, quality and governance.
The opportunity
We are establishing our Aged Care On-site Pharmacist program across Sydney metropolitan aged care homes and are looking for credentialed pharmacists to take up these roles as they come online.
What you will bring
Current general registration as a pharmacist with AHPRA, with no conditions on practice
ACOP credentialing through a completed APC-accredited aged care on-site pharmacist training program, as required for all pharmacists participating in the measure from 1 July 2026
Experience in aged care, community or hospital pharmacy, and comfort working alongside prescribers
Clear, confident communication with nurses, GPs, families and facility management
A current driver's licence and willingness to travel between homes within Sydney metro
National police check and NDIS Worker Screening Check, or willingness to obtain

You will join an established national team of highly experienced pharmacists with clinical supervision, peer review and structured governance behind you, purpose-built medication review technology, and ongoing professional education delivered through our own registered training organisation. We offer full time and part time arrangements, with days able to be structured across one or several homes to suit you.
