NDIS TEAM LEADER
About the Role
Apex Support Services is seeking a proactive, compassionate, and experienced Team Leader to oversee the delivery of high-quality disability support services across the Greater Sydney region and surrounding areas.
This is a mobile, field-based role that combines leadership, participant support, staff development, and operational oversight. Reporting to the Operations Team, you will play a key role in participant onboarding, care planning, staff supervision, quality assurance, and ensuring all supports are delivered in accordance with the NDIS Practice Standards and NDIS Code of Conduct.
KEY DUTIES & RESPONSIBILITIES
Participant Intake & Care Planning
Part-take in participant intake meetings and assessments.
Develop, implement, and regularly review individualised support plans, risk assessments, and care documentation.
Work collaboratively with participants, families, guardians, and allied health professionals to achieve participant goals.
Staff Induction, Training & Leadership
Onboard and Support Disability Support Workers.
Provide ongoing coaching, performance feedback, and field-based supervision.
Promote best practice, person-centred supports, and compliance with organisational policies.
Rostering & Operational Coordination
Assist with rostering, workforce planning, and participant-to-support worker matching.
Monitor service delivery to ensure continuity of supports and efficient resource allocation.
Respond to operational issues and provide leadership to the support team.
Participant Support & Quality Assurance
Provide direct support to participants when required, including in-home, community access, Supported Independent Living (SIL), and respite services.
Conduct regular quality assurance visits to ensure supports are delivered safely and effectively.
Support participants with complex needs while promoting independence, dignity, and choice.
Compliance & Documentation
Maintain accurate participant records and case notes.
Complete incident reports, restrictive practice documentation, risk assessments, and care plan reviews.
Ensure all documentation meets NDIS Quality and Safeguards Commission requirements and Apex Support Services policies.
Stakeholder Engagement
Build positive relationships with participants, families, Support Coordinators, allied health professionals, and other key stakeholders.
Represent Apex Support Services professionally and contribute to continuous service improvement.
QUALIFICATIONS & EXPERIENCE
Essential
Diploma or Bachelor's degree in Community Services, Disability, Social Work, Psychology, Human Services, Nursing, or a related field (or equivalent industry experience).
Minimum 1–2 years' experience working within the disability sector.
Demonstrated experience supporting people with physical disabilities, intellectual disabilities, autism, acquired brain injury, psychosocial disability, and other complex support needs.
Experience developing care plans, conducting participant intakes, and supervising or mentoring support staff.
Sound knowledge of the NDIS Practice Standards, NDIS Code of Conduct, and person-centred support principles.
Current competency in medication administration, manual handling, infection control, and the use of mobility equipment such as hoists and transfer aids.
MANDATORY REQUIREMENTS
Current NDIS Worker Screening Check.
Current Working with Children Check.
Current First Aid and CPR Certificate.
Police Check
Current Australian Driver Licence and reliable, fully insured vehicle.
Willingness to travel across Greater Sydney and surrounding regions as required.
