Clinical Nurse Consultant Chronic Disease

Your opportunity

The position will sit within General Practice at the Charleville Health Clinic working largely within the Rural and Remote Chronic Disease Model of Care to provide evidence based coordinated care to individuals and groups of chronic disease clients including those with type 2 diabetes, cardiovascular disease, chronic respiratory disease, and renal disease and other long-term conditions.

The Clinical Nurse Consultant practices autonomously to provide specialist clinical nursing services and advice in collaboration with the multi-disciplinary team within the Rural and Remote Chronic Disease Model of Care, with a key focus on the Aboriginal and Torres Strait Islander people. The Clinical Nurse Consultant also mentors Practice Nurses in relation to the nursing aspects of chronic disease management.

Charleville is a designated Remote Area Nursing Incentive Package (RANIP) site in accordance with the Nurses and Midwives (Queensland Health) Award – State 2015. This attracts additional entitlements which may include:

• An annual isolation bonus paid annually!
• Return airfares to the nearest provincial city in conjunction with leave!
• Additional annual leave per annum!
