Join Ambition Health Group, a trusted and registered provider under NDIS, My Aged Care, TAC, WorkSafe & NIISQ, delivering person-centred care across homes, community settings, and Specialist Disability Accommodation (SDA).
We’re seeking compassionate Disability Support Workers to assist participants with daily living, personal care, and community access in Toowoomba. You’ll support people living with mental illness, intellectual, or physical disabilities — helping them live independently and achieve their personal goals.
About the roleWe are seeking an experienced Disability Support Worker to join our support team and provide consistent, reliable support.
This is a role suited to a confident and experienced support worker who is comfortable working with people with complex mental health and alcohol and other drug (AOD) support needs.
Support requiredWe require support that is consistent, respectful, structured and focused on maintaining wellbeing, safety and community participation.
Support requirements include:
Assistance with attending appointments and engaging with community-based services.
Monitoring for changes in presentation, behaviour or wellbeing and appropriately escalating concerns.
Following established risk-management and support procedures.
Maintaining clear and appropriate professional boundaries.
Communicating effectively with the relevant support team and external professionals.
Following required escalation procedures where required.
We are specifically seeking a support worker with demonstrated competency and confidence in supporting people with:
Complex mental health needs.
Behaviours or presentations that may change unexpectedly.
Community-based support and appointment attendance.
You will need to be calm, reliable and observant, with the ability to recognise changes in presentation and respond appropriately.
The successful applicant will demonstrate:
Strong professional boundaries and ethical practice.
Excellent observation and communication skills.
The ability to remain calm and professional in challenging situations.
Confidence following risk-management and escalation procedures.
Reliability and consistency.
A person-centred and non-judgemental approach.
The ability to work independently while communicating effectively with the broader support team.
A genuine commitment to building trust and rapport over time.
Regular, ongoing shifts.

Applicants must have:
Demonstrated experience supporting people with complex mental health needs.
Previous experience managing or responding appropriately to changes in presentation and risk.
Current Police check/NDIS Worker Screening Check/Blue Card/First Aid/CPR/Flu vaccination/100 pts of ID].
A current Australian driver’s licence and reliable vehicle.
