About the Role We're looking for an experienced Part-Time Chronic Disease Management (CDM) Nurse to join our Leichhardt Medical Centre. In this rewarding role, you'll work closely with GPs and patients to support the prevention and management of chronic conditions. You'll play a vital role in empowering patients to take control of their health, improving long-term outcomes, and reducing avoidable hospital admissions. Key Responsibilities Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs). Conduct health assessments and identify patients who may benefit from chronic disease management. Support and educate patients to better understand and manage their health conditions. Coach patients on sustainable health behaviour changes. Coordinate referrals and ongoing care with multidisciplinary healthcare providers. Partner with GPs to promote preventative healthcare initiatives across the practice. Increase patient engagement in chronic disease management programs. Ensure care plans are reviewed and updated in line with clinical requirements. Complete all administrative tasks associated with CDM appointments accurately and efficiently. About You To be successful in this role, you will have: Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN). Demonstrated experience in Chronic Disease Management within a general practice setting. Experience completing health assessments (highly regarded). Excellent organisational and time management skills. Strong communication and interpersonal skills with a patient-focused approach.
