Overview
The Eyre and Far North Local Health Network (EFNLHN) is seeking a committed and person-centred professional to join the Streaky Bay and Ceduna Community & Allied Health team as a Case Coordinator or Care Partner - Support at Home. This full-time, ongoing opportunity is open to both ASO3 Case Coordinators and Enrolled Nurses, with the appointment and classification determined by the skills, qualifications and experience of the strongest candidate.

Working across Streaky Bay and Ceduna, you'll coordinate and deliver home support services under the Commonwealth Home Support Program (CHSP) and the new Support at Home program. Your day-to-day will include:
