Position Overview
We are seeking an Enrolled Nurse to join our Case Management and Care Coordination team in a Permanent Part-Time role. You will play a crucial role in delivering clinical nursing assessment, care planning, and coordination of support services for clients within their homes. Working collaboratively with multidisciplinary teams, you will ensure clients receive integrated, coordinated care that supports their independence and wellbeing. Key Responsibilities
· Conduct clinical nursing assessments and contribute to individualised care plans aligned with client goals and NDIS plans
· Provide case management support including assessment, planning, implementation, and review of client care needs
· Coordinate care across multiple service providers and health professionals to ensure seamless, integrated support
· Administer medications and monitor clients for adverse reactions, side effects, and health changes
· Perform clinical nursing interventions including wound care, vital signs monitoring, catheter care, and continence management
· Support chronic disease management and monitor health conditions with appropriate escalation
· Develop and maintain accurate, comprehensive care plans that reflect client preferences and clinical needs
· Document all clinical assessments, observations, interventions, and client progress in compliance with relevant standards
· Liaise with GPs, specialists, allied health professionals, and other service providers to facilitate coordinated care
· Provide health education to clients and carers, promoting self-management and client independence
· Identify and escalate clinical risks, safeguarding concerns, and changes in client condition to appropriate clinical leads
· Support clients with personal care, mobility assistance, and activities of daily living as required
· Build and maintain trusting, respectful relationships with clients, families, carers, and the broader care team
· Contribute to care coordination meetings and provide clinical input on client progress and needs
· Ensure compliance with NDIS Quality and Safeguarding Framework, Support at Home program requirements, and all relevant legislation
· Demonstrate flexibility and willingness to travel within the local service area to support clients in their homes
Selection CriteriaEssential Qualifications & Registration:
· Current AHPRA registration as an Enrolled Nurse
· Current Australian driver's licence and access to a reliable vehicle
· Current National Police Check (or willingness to obtain one)
· Valid work rights in Australia
· Knowledge of NDIS principles and Support at Home program requirements (or willingness to develop)
Essential Experience & Capabilities:
· Demonstrated experience in nursing practice with strong clinical assessment and observation skills
· Ability to contribute to care coordination and case management processes
· Proficiency in medication administration and clinical nursing interventions
· Ability to commit to a minimum of 15 hours per week with flexibility for hours to increase
· Flexibility to work across all shifts, including night shifts, and to travel within the local service area
· Strong communication and interpersonal skills with ability to build rapport with diverse client groups
· Ability to work collaboratively within multidisciplinary teams and liaise with multiple service providers
· Excellent documentation and clinical reasoning abilities
· Strong time management and organisational skills
· Demonstrated commitment to person-centred, outcomes-focused care delivery
Highly Desirable Experience:
· Experience in community nursing, aged care, or disability support services
· Previous experience with NDIS clients or in NDIS service delivery
· Experience in case management or care coordination roles
· Knowledge of Support at Home programs
· Experience working with clients with chronic conditions, disabilities, or complex care needs
· Familiarity with electronic health records and care management systems
Pay: $52,880.00 – $100,071.71 per year
