About the role
Path To Independence is seeking an experienced and dedicated Disability Support Worker to provide high-quality support to a participant living with an acquired brain injury and complex physical support needs.
This role is ideal for someone with a strong background in disability support, rehabilitation, nursing, aged care, allied health assistance, or complex care who is confident supporting participants with significant mobility and personal care requirements.
About the RoleThe successful applicant will provide:
Personal care and activities of daily living support.
Assistance with mobility, transfers, positioning, and physical support needs.
Support in accordance with rehabilitation and therapy recommendations.
Medication assistance and health monitoring as directed.
Community access and appointment support.
Documentation, progress notes, and effective communication with the broader support team.
Support using positive behaviour support strategies when required.
Previous experience supporting individuals with acquired brain injury, neurological conditions, spinal injury, or other complex disabilities.
Demonstrated experience providing high-level personal care.
Current NDIS Worker Screening Check.
Current First Aid and CPR certification.
Valid driver's licence and reliable vehicle.
Strong communication and report-writing skills.
Ability to remain calm, professional, and person-centred in complex situations.
Disability Support Worker experience.
Nursing, Assistant in Nursing, Allied Health Assistant, Rehabilitation Support Worker, or similar healthcare experience.
Knowledge of manual handling techniques and use of mobility equipment.
Experience supporting participants with complex medical, cognitive, and behavioural support needs.
