What your day could look like

As a Clinical Nurse – Case Manager, you'll help older people transition from hospital to the next stage of their recovery with confidence. Working closely with patients, families and healthcare professionals, you'll coordinate care that improves wellbeing, supports independence and delivers positive outcomes.

Every day, you'll contribute by:

• Coordinating individual care plans that support recovery and independence.
• Connecting patients, families and providers to achieve shared goals.
• Providing clinical leadership and guidance within a multidisciplinary team.
• Driving quality improvements that enhance patient outcomes and experiences.

Here's what will help you thrive in this role:

• Current registration as a Registered Nurse.
• Experience in case management and multidisciplinary care coordination.
• Strong assessment, clinical reasoning and decision-making skills.
• Excellent communication and relationship-building abilities.

You'll stand out if you have postgraduate study relevant to nursing practice and experience working with older people in rehabilitation or transition care settings.
