The Role
We’re seeking an experienced Team Leader Care Management to lead our team of 11 Care Partners across 600 aged and disability home care clients.
This is a genuine people-leadership role. You’ll provide coaching, direction and support to your team while ensuring care management practice is consistent, compliant and outcomes focused.
You’ll thrive in this role if you’re a confident people leader who enjoys creating a high-performing team culture while keeping a strong focus on operational performance, quality and continuous improvement.
