The Role

We’re seeking an experienced Team Leader Care Management to lead our team of 11 Care Partners across 600 aged and disability home care clients.

This is a genuine people-leadership role. You’ll provide coaching, direction and support to your team while ensuring care management practice is consistent, compliant and outcomes focused.

Key Responsibilities

• Lead, coach and support the Care Management team, fostering a positive, collaborative and high-performing culture.
• Provide regular supervision and professional development to build staff capability.
• Manage team capacity, workflow and priorities to ensure timely, consistent care management.
• Monitor performance and care management KPIs, identifying opportunities to improve quality, efficiency and outcomes.
• Contribute to reporting on outcomes, risks and improvement opportunities.
• Support the implementation of care management policies, procedures and frameworks.
• Contribute to quality assurance activities, including audits, reviews and improvement initiatives.
• Ensure practices align with Support at Home, NDIS requirements and organisational policies.
• Guide the team in managing complex client needs, risk identification, escalation and advocacy.
• Oversee the documentation, investigation and resolution of client incidents and complaints.
• Work collaboratively with intake, scheduling, clinical and finance teams to support a coordinated client journey.
• Support the team to build strong relationships with clients, carers, families and external providers.

About You

You’ll bring:

• Significant experience in care management, case management or service coordination within aged care and/or disability services.
• Demonstrated leadership experience, with a strong track record of developing staff capability and team performance.
• Sound knowledge of the Support at Home program, NDIS and person-centred care principles.
• Strong understanding of the Aged Care Quality Standards and NDIS Practice Standards.
• Experience managing and investigating incidents and complaints within a regulated service environment.
• Strong organisational, problem-solving and time-management skills, with the ability to manage competing priorities.
• Excellent communication and stakeholder engagement skills, with the ability to work effectively with clients, families, staff and external partners.
• Confidence using client management systems and reporting tools.
• A high level of attention to detail and a commitment to accurate documentation.
• A current NDIS Worker Screening Check and Working with Children Check, or willingness to obtain.

Desirable

• Empathy, cultural sensitivity and an understanding of the unique needs of Holocaust survivors and the Jewish community.
• A Diploma in Community Services, Aged Care, Case Management or a related discipline, or equivalent experience.
• A clinical background.
