The Opportunity
GOALS+ is a housing and support program for young people aged 16–21 who are leaving care, experiencing homelessness or navigating a critical transition.
As a Case Manager, you will provide strengths-based, trauma-informed case management and practical support. Working alongside young people, care teams and community services, you will develop individual plans and connect participants with housing, education, employment, health and wellbeing supports.
You will help young people build practical life skills, manage risks and work towards stable housing and greater independence. This is a meaningful opportunity to support young people through a critical stage of their lives and help them build a safe and sustainable future.
About You
You are an empathetic and resilient practitioner with experience supporting young people with complex needs. You can provide trauma-informed case management, manage risk, respond calmly during crises and work collaboratively with care teams and community services.
You Will Bring
Experience in youth case management, housing, homelessness or related services.Strong assessment, communication, advocacy and documentation skills.A Diploma-level qualification in Social Work, Youth Work or a related discipline.
A relevant Bachelor’s degree and experience in housing or homelessness programs would be highly regarded.
