Role Responsibilities:

• Coordinate formal and informal supports

• Manage clients with complex needs and handle crisis situations

• Collaborate with a Multidisciplinary Panel

• Engage with Mental Health, Justice System, Housing, and Health sectors

• Undertake clerical administration and report writing

• Network and communicate with key stakeholders

• Work independently

• Work in a sensitive, complex and demanding environment with a high degree of autonomy and self-management.

Qualifications & Experience:

• Minimum of Certificate IV in Community Services or related discipline

• Minimum 2 years' experience working with participants with moderate to complex mental health issues, high medical needs, and/or behavioural support needs

• Experience in Forensic Disability or Forensic Mental Health is a plus

• Demonstrated in-depth knowledge of the NDIS and experience working with its participants

• Valid Driver's License with a suitable, insured vehicle

• A current Working with Vulnerable People registration

• Recent police check (or willingness to obtain one)
