Established in 1982, Alzheimer’s WA (AWA) provides care, support, education, advocacy and information to assist people living with dementia, as well as their families and carers.

Our services include dementia education and training, consultancy, advocacy, partnerships, CHSP, Support at Home and day and overnight respite across Perth, Mandurah and Albany.

About the Role

We are seeking an experienced and organised House Coordinator to join our team in Albany and support the coordination and delivery of dementia-focused services.

Working closely with the House Manager, Registered Nurses, Support Workers and other teams, you will help ensure clients receive safe, high-quality and well-coordinated services.

Key Responsibilities

• Support the onboarding of clients, including service agreements, budgets and support arrangements.
• Maintain accurate client records and ensure aged care funding and compliance requirements are met.
• Assist with care plans, risk assessments and coordination of support services.
• Provide administrative and non-clinical support to Registered Nurses and the wider team.
• Coordinate communication between clients, families and Support Workers.
• Maintain documentation including care records, incidents, restrictive practices and progress notes.
• Monitor client information, budgets and service documentation in the CRM.
• Support continuous improvement and quality service delivery.

About You

To be successful in this role, you will have:

• Experience in aged care, community services, health or a similar environment.
• Experience coordinating services and supporting clients, particularly people living with dementia.
• Strong administrative, organisational and time management skills.
• Excellent communication and interpersonal skills.
• Strong attention to detail and the ability to manage competing priorities.
• An understanding of aged care funding, compliance and service delivery requirements.
• Experience using CRM systems and Microsoft Office.
• A current National Police Clearance or willingness to obtain one.
• Knowledge or experience with the Support at Home Program and CHSP.
• Experience with My Aged Care and aged care funding requirements.
