About the role
Are you an experienced nurse who enjoys getting to know your patients and making a genuine difference to their long-term health?
We are looking for a Practice Nurse – Chronic Conditions Management to join our friendly and established general practice in Port Macquarie.
This role would suit a nurse who is passionate about preventative healthcare, enjoys working closely with GPs and allied health professionals, and takes pride in helping patients better understand and manage their health.
You will have the opportunity to build meaningful relationships with patients and provide ongoing support to those living with chronic and complex health conditions.
Your day will involve a combination of clinical care, patient education, care coordination and health promotion.
You will be responsible for:
Developing and reviewing Patient-centred Care Plans in collaboration with patients and GPs.
Completing health assessments and identifying patients who may benefit from additional preventative care.
Supporting patients to set realistic goals around their health and wellbeing.
Providing education and encouragement around lifestyle changes and self-management.
Monitoring patients and following up on their progress over time.
Coordinating care with allied health providers, specialists and other members of the patient's healthcare team.
Working alongside our GPs to ensure patients receive well-coordinated and comprehensive care.
Identifying patients who may be eligible for Chronic Conditions Management and other preventative health services.
Maintaining clear and accurate clinical records.
Completing the necessary Medicare and practice documentation associated with Patient-centred Care Planning and health assessments.
There is also scope to contribute ideas and help further develop the practice's approach to chronic conditions management and preventative healthcare.
You will be a qualified Registered Nurse or Enrolled Nurse with current AHPRA registration and a genuine interest in primary healthcare.
Ideally, you will have experience in:
Chronic disease or chronic conditions management.
Chronic conditions management care planning
Health assessments and preventative healthcare.
General practice or another primary healthcare setting.
You will also bring excellent communication skills, sound clinical judgement and the ability to work both independently and as part of a close-knit team.
Most importantly, we're looking for someone who is approachable, organised and genuinely interested in their patients. You should enjoy taking the time to listen, explain and encourage, particularly when helping patients make changes that can improve their long-term health.
