Every day, people experiencing homelessness face barriers that can seem impossible to overcome. Poor health, social isolation, trauma and disconnection from services can leave people without the support they need to move forward.
At Better Health Network, we're creating a different response.
We're looking for a passionate and experienced Case Manager - Complex Care to join our innovative Pre-Housing Homelessness Program and walk alongside some of our community's most vulnerable people as they navigate pathways to stability, wellbeing and housing.
This is more than a case management role.
It's an opportunity to build trusted relationships, advocate for meaningful change and become part of a multidisciplinary team delivering a health-led model of care that brings together housing, health and community services to achieve lasting outcomes for people who need it most.
About The Role
As a Case Manager - Complex Care, you'll be at the heart of a program designed to support people experiencing homelessness and complex disadvantage before they enter long-term housing.
You'll work directly with individuals facing significant barriers, helping them access healthcare, housing, mental health support, alcohol and other drug services, legal assistance and community connections.
Through assertive outreach, practical support and genuine engagement, you'll help people take steps towards improved health, independence and a more stable future.

About You
You are someone who genuinely enjoys working with people and believes everyone deserves to be treated with dignity, respect and compassion.
You bring a positive attitude, strong initiative and a willingness to go the extra mile to help people achieve meaningful outcomes. You're a team player who thrives in collaborative environments, builds strong relationships with ease and isn't afraid to roll up your sleeves when challenges arise.
To Be Successful, You'll Bring
Relevant tertiary qualifications in Social Work, Psychology, Community Services or a related field. Demonstrated case management experience with vulnerable and marginalised adults, including people experiencing homelessness, poor health or service disconnection. Strong assessment, planning, coordination and advocacy skills. Ability to conduct outreach and build rapport with clients in diverse community settings. Sound knowledge of the social and community services sector and the factors that influence health and wellbeing. Clear written and verbal communication, strong organisation and accurate documentation skills. Critical thinking, sound judgement and the ability to respond effectively in crisis situations. Ability to build productive relationships with health, housing and community service partners.

Location
This role is primarily based at Level 2, Victorian Pride Centre, 79-81 Fitzroy Street, St Kilda, with outreach and travel across community settings and other BHN locations as required.
