About the role
Merri Outreach Support Service (MOSS) has been supporting people experiencing or at risk of homelessness since 1989. Guided by our vision of striving to end homelessness, sustaining housing and strengthening communities, we help people access housing, support services and opportunities that improve wellbeing and social inclusion. Through our Social Support Groups Program, we work alongside older people and people with disability who may be experiencing social isolation, health challenges, housing instability or other complex needs, helping them remain connected, independent and engaged in their community.
We are seeking a passionate and skilled Case Manager – Social Support Groups to join our team. This rewarding role supports older people and people with disability to identify and achieve their goals, access services, build social connections and maintain their independence. Working closely with participants, carers and group workers, you will play a key role in enhancing wellbeing and creating positive outcomes for people in our community.
Working across sites in Brunswick, Nillumbik, Banyule and Darebin, you will provide person-centred case management, assessment, care planning, advocacy, referrals, service coordination and practical support. You will build trusted relationships with participants, collaborate with a range of community and health services, and support people to remain active, connected and living safely in their own homes for as long as possible.
We are looking for someone with experience in aged care, disability, community services, social work or a related field who is committed to person-centred practice, inclusion and social justice. You will have strong communication, engagement and advocacy skills, a sound understanding of My Aged Care and/or the NDIS, and a genuine commitment to supporting diverse communities. A relevant qualification in Community Services, Social Work, Community Development or a related discipline is highly desirable.
