About the role
healthAbility, as the name suggests, plays a unique and valuable role in the health system. We work to address the root causes of vulnerability, prevent disease and promote lifelong health outcomes, empowering people in our community to live their best life.
Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

The Reconnect Program supports young people aged 12-18 years who are at risk of homelessness and live, work, study or play in Banyule or Nillumbik areas. The Reconnect Counsellor & Case Manager uses a strengths-based, holistic approach to support young people, their families, carers and other networks to prevent homelessness, create opportunities to sustain safe home environments, improve life-skills and improve overall wellbeing.
The Reconnect Counsellor & Case Manager is actively involved in community networks, with schools and other organisations to build awareness of youth homelessness and build capacity among parents, carers, educators and other professionals. The role uses case management and counselling frameworks and holds a caseload of 20-30 young people per year.
The Reconnect program sits within the Children, Youth and Family team at healthAbility, and is co-located at the Nillumbik Youth Hub, with a substantial Outreach focus. The role is part time (0.6 FTE) until June 30, 2027.
If you have a qualification in Social Work, Youth Work, or similar, and demonstrated experience supporting young people, especially through case management, counselling or outreach roles, we would love you hear from you. We are looking for a well-rounded professional who is passionate about young people and youth homelessness, who can work independently and contribute to a multidisciplinary team.
Experience in the homelessness sector, with LGBTIQA+ and neurodiverse young people, in group facilitation, in family mediation and parent/carer capacity building would be highly regarded.
