Social Worker, Grade 1 - Casey Hospital

We are looking for a Grade 1 Social Worker to join our Adult Acute Social Work team at Casey Hospital in a full-time position.

This is an exciting opportunity to build your clinical skills in a busy acute hospital environment while being surrounded by an experienced, supportive and collaborative Social Work team. You will work alongside patients, families, carers and multidisciplinary teams to understand the broader factors impacting a person's health and wellbeing and help navigate safe, person-centred care and discharge planning.

As a Grade 1 Social Worker, you will manage a varied clinical caseload while continuing to develop your psychosocial assessment, intervention and care planning skills. No two days will be quite the same. You will:

• Complete comprehensive psychosocial assessments and develop patient-centred care plans
• Support patients and families through complex discharge planning and transitions of care
• Work collaboratively with Social Work, Allied Health, medical and nursing colleagues as part of a multidisciplinary model of care
• Liaise with internal and external services to connect patients with appropriate supports and resources
• Contribute to rostered service delivery requirements, including support to the Emergency Department and leave coverage
• Deliver care informed by current evidence, professional standards and a commitment to safe, person-centred practice

At Monash Health, Social Workers play an important role in supporting patients and families through some of the most challenging and significant moments in their healthcare journey.

Our Social Work Service provides comprehensive psychosocial assessment, counselling, advocacy, information and support, connection to community resources, and discharge planning. Working as integral members of multidisciplinary teams, our Social Workers bring a valuable psychosocial perspective to patient care, particularly where there is social complexity or vulnerability.

The service operates Monday to Friday during standard business hours, with weekend coverage provided on a rostered basis across Casey Hospital, Monash Medical Centre Clayton, Dandenong Hospital and the Victorian Heart Hospital. An on-call Social Work service also supports our acute sites.

• Strong communication, interpersonal and organisational skills

• A current Victorian Driver's Licence
