Clinical Aged Care Assessor

Part time or casual positions available

Your responsibilities:

•              Travel to complete face to face assessments within Northern Sydney region, Western and South East Sydney (depending on where you are based) and use your clinical expertise to assess the needs of older Australians

•              Conducting triages through phone

•              Undertaking assessments with a strength-based approach, with a good understanding of Wellness, Reablement and increasing independence for older Australians.

•              Conducting client reviews to keep up to date with client’s changing needs and circumstances

•              Completing client assessments and support plans within the deadlines provided

•              Connecting the client with service providers and community support organisations local to them

To be successful for this role you must:

•              Have a tertiary degree in allied health such as occupational therapist, social worker, physiotherapist, registered nurse or any other allied health degree.

•              Have experience working with older people.

•              High level computer literacy, including proficiency of typing and MS Office programs.

•              Have your own car and prepared to travel to deliver face to face assessment services in the client’s own home

•              Hold a full current driving licence

Desirable skill set includes:

•              Experience in assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services

•             Demonstrated ability to work independently, as well as function as a member of a team.

•              Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach

•              Proactive, self-motivated person with the capacity to follow due process.

•              Good team member, with excellent communication skills

•              Second language is a distinct advantage
