About the role
Real Community Services (RCS) is seeking a compassionate, reliable, and experienced Disability Support Worker to provide high-quality, person-centred support to a participant living with an intellectual disability and complex health needs.

In this rewarding position, you will provide individualized support tailored to the participant's goals, preferences, and support requirements. Duties include assistance with personal care, diabetes management routines, community access, and daily living activities.
The participant requires consistent support to manage Type 1 Diabetes, epilepsy (including Grand Mal and absence seizures), and activities of daily living. As a trusted member of the support team, you will work collaboratively with the participant, their support network, and allied professionals to promote dignity, choice, independence, and quality of life.
Assist the participant to wake and prepare for the day in a gentle and supportive manner.
Support blood glucose level (BSL) monitoring in accordance with established diabetes management plans.
Assist with insulin-related routines within the worker's scope of practice and the participant's approved support plan.
Provide personal care assistance and support with daily hygiene.
Offer patient, respectful verbal prompts and guidance as required.
Support the participant to prepare for and attend community outings.
Assist the participant to understand and manage their spending budget for planned activities.
Promote informed choice, independence, and confidence while engaging in community activities.
Provide support with mobility, safety, and community participation.
Foster positive social engagement while maintaining awareness of the participant's individual support needs and preferences.
Support BSL monitoring and diabetes management routines.
Assist with personal care and daily living tasks.
Prepare bathing arrangements and provide supportive prompts regarding personal hygiene.
Maintain a calm, reassuring environment, particularly during periods of increased anxiety, tremors, or discomfort around unfamiliar people.
Monitor participant wellbeing and respond appropriately in accordance with established epilepsy and health-management protocols.

To be successful in this role, you will demonstrate:
Previous experience supporting people with disability and providing personal care.
Confidence supporting individuals with complex health needs, including diabetes and epilepsy.
Experience following individualized support plans and health-management routines.
The ability to remain calm, attentive, and responsive in situations involving seizures, tremors, or changing support needs.
Strong interpersonal skills and the ability to build trusting, positive relationships.
A commitment to promoting participant choice, dignity, and independence.
Reliability, professionalism, and a person-centred approach to support.
Excellent communication skills and the ability to work collaboratively within a multidisciplinary team.
