Key Responsibilities

• Provide high-quality, person-centred support to people with disabilities.
• Lead by example and provide guidance and support to Support Workers during shifts.
• Assist with the implementation and monitoring of individual support plans.
• Support customers with medication administration, complex care and daily living activities.
• Ensure accurate documentation, shift handovers and customer records are maintained.
• Promote customer choice, independence, dignity and community inclusion.
• Foster a safe, inclusive and empowering environment for customers and team members.
• Work collaboratively with Team Leaders and the wider multidisciplinary team to achieve positive customer outcomes.

About You

• Passionate about supporting people with disability to achieve their goals.
• Previous experience in disability support, with leadership or mentoring experience highly regarded.
• Strong communication and interpersonal skills with the ability to build positive relationships.
• Confident supporting customers with complex care needs and behaviours of concern.
• Organised, proactive and able to use initiative in a fast-paced environment.
• Committed to delivering person-centred, high-quality support.
• Reliable, professional and a positive role model for your team.
