Where you'll be working
Based at Blacktown Hospital, you will join Western Sydney Local Health District (WSLHD) and play a key role in developing and coordinating the Osteoporotic Refracture Prevention Service (ORP).
Working in partnership with endocrinology, hospital clinicians, general practitioners and community providers, you will help strengthen pathways that support patients following minimal trauma fractures and improve access to timely osteoporosis assessment and treatment.Work across Blacktown and Mount Druitt Hospitals, collaborating with equivalent services across WSLHD.Be part of a multidisciplinary environment focused on person-centred, evidence-based care.Build strong partnerships across hospital, primary care, private and community services.Contribute to a service focused on reducing preventable re-fractures and improving long-term health outcomes.Support a culture of collaboration, innovation, quality improvement and clinical excellence.
What You'll Be Doing
As the Fracture Liaison Coordinator, you will coordinate the ORP service and clinic in collaboration with the endocrinologist, providing specialist clinical support to people identified as being at risk of osteoporosis and re-fracture. You will:
Coordinate and evaluate the ORP, including patient recruitment, assessment, education, treatment pathways and long-term follow-up.Provide expert clinical advice and develop individualised care management plans for patients with complex health needs.Empower patients through education and coaching on osteoporosis, re-fracture risk, chronic disease management and self-management.Lead quality improvement, research, data collection and evaluation activities to strengthen service outcomes and clinical practice.Develop strong referral pathways and partnerships with healthcare providers to ensure timely access to investigations, treatment and multidisciplinary support.

Essential Criteria
Qualification: As per Allied Health Profession Registration: Current AHPRA Registration: Allied Health Professional
Selection Criteria
Relevant Allied Health qualifications with significant post graduate clinical experience involving populations with musculoskeletal conditions and/or chronic diseases.Demonstrated interest in musculoskeletal health care including extensive knowledge of the contemporary issues around osteoporosis and its management.Well-developed understanding of the concepts of chronic care and its application with people and communities where chronic care is required.Demonstrated experience in planning and coordinating multi-disciplinary care and the capacity to work cooperatively and effectively with medical, allied health professionals and consumers with the ability to provide expert clinical education and training.Demonstrated highly developed interpersonal, written and electronic communication skills and the ability to build and maintain relationships.Proven ability to take initiative, make decisions, and work both autonomously and in a close team environment.Proficiency in Information Technology such as Microsoft applications, email, clinical software packages and an ability to manage and maintain database information with accuracy, proficiency and attention to detail.
