Omnia Community Health was established through the merger in January 2026 of Sunbury and Cobaw Community Health and Nexus Primary Health.  Bringing together the deep community connections, shared values and specialist expertise of both organisations, Omnia builds on more than 50 years of experience delivering health and wellbeing services within the not-for-profit sector.

We offer a broad range of services across the lifespan including general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing.  These services are delivered from our sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach across surrounding communities.

About the Role

The Exercise Physiologist is responsible for delivering comprehensive assessment and evidence-based intervention services for adults with diverse needs, including people living with disability and chronic health conditions. The role supports clients to achieve their functional goals through individual and group-based exercise programs, health education, and capacity-building initiatives. Working as part of a multidisciplinary team, the Exercise Physiologist contributes to person-centred care through interdisciplinary collaboration, service development, and continuous quality improvement activities.

About You

You:

• Can demonstrate high quality, person-centered and evidence-based assessment and intervention to group and individuals of the community. 
• Have strong administrative skills, including recording contracts via TrakCare system, management of client health records and utilisation of My Ages Care and NDIS service systems. 
• Coordinate client care through effective communication and collaboration with health practitioners, care providers and family members, including the provision of timely feedback and professional reports. 
• Can work both independently and collaboratively across the exercise physiology team. 
• As a Grade 2, you have demonstrated experience providing clinical supervision to students, Grade 1 Exercise Physiologists, Allied Health Assistants and volunteers. 

Requirements

• Accredited Exercise Physiologist with Exercise and Sport Science Australia (ESSA)
• Current National Criminal History Check, NDIS Work Screening, Working with Children's Check and Evidence of right to work in Australia 
• Current Victorian Driver's Licence
