About the role
Oasis Care Centre is a registered NDIS provider supporting participants throughout Victoria. We provide personalised and collaborative care to assist people living with disability to achieve greater independence and participate meaningfully in their communities.
Our services include assistance with daily living, personal care and safety, household support, social and community participation, transport support and other individualised supports based on the needs and goals of our participants.
We are currently seeking an experienced and compassionate Disability Carer to join our growing team on a full-time basis.
About the RoleAs a Disability Carer, you will provide individualised care and support to participants living with disability. You will work closely with participants, their families and other members of the care team to provide safe, respectful and person-centred support in accordance with each participant's individual needs and support plan.
Key ResponsibilitiesYour duties will include:
Providing personal care and assistance with activities of daily living including showering, dressing, grooming, toileting and mobility;
Assisting participants with meal preparation, eating and other daily living requirements;
Supporting participants to develop and maintain independence and daily living skills;
Assisting participants to access and participate in social, recreational and community activities;
Providing companionship and emotional support to participants;
Assisting participants with household tasks and maintaining a safe and comfortable living environment;
Providing support with mobility, transfers and the use of mobility or other assistive equipment where required;
Assisting participants with transport and attendance at appointments, activities and community events;
Following individual participant support plans and established care procedures;
Monitoring participants and reporting changes in their physical, emotional or behavioural wellbeing;
Maintaining accurate records, progress notes and other required documentation;
Working collaboratively with participants, families, support coordinators, healthcare professionals and other members of the care team;
Supporting participants to achieve their individual NDIS goals;
Promoting participant choice, dignity, privacy, independence and community inclusion;
Following NDIS requirements and organisational policies and procedures including workplace health and safety, incident reporting and safeguarding requirements; and
Performing other disability support and care duties reasonably required in connection with the position.
To be considered for this position, applicants must have:
A minimum of one year of relevant work experience as a Disability Carer, Disability Support Worker, Personal Care Assistant, Aged or Disabled Carer or in another closely related care or support role;
A minimum Certificate II or Certificate III qualification in Individual Support, Disability, Community Services, Aged Care or another closely related field;
Demonstrated experience providing personal care and daily living support to people living with disability;
An understanding of person-centred care and the importance of promoting participant choice, dignity and independence;
Good communication and interpersonal skills;
The ability to work effectively both independently and as part of a multidisciplinary care team;
Good organisational and record-keeping skills; and
A genuine commitment to providing safe, respectful and high-quality care to people living with disability.
Applicants must also be willing and able to satisfy all applicable NDIS worker screening, police, working with children and other mandatory employment requirements relevant to the position.
