NDIS Support Coordinator

About the role

The Support Coordinator is responsible for assisting NDIS participants in understanding and implementing their NDIS plans, connecting them with appropriate services, and ensuring they can access the supports required to achieve their goals. This role involves working collaboratively with participants, service providers, and other key stakeholders to enhance participant independence, community engagement, and service delivery outcomes.

Key responsibilities

• Assist participants in understanding their NDIS plans and available supports

• Develop and coordinate individualised service and support plans

• Identify appropriate service providers and facilitate service agreements

• Monitor service delivery to ensure participant goals are being met

• Engage effectively with participants, families, and service providers

• Maintain professional and collaborative relationships with internal and external stakeholders

• Ensure clear, timely, and accurate documentation of all participant interactions

• Advocate on behalf of participants to ensure access to necessary supports

• Foster strong, respectful relationships with participants and their families

• Manage caseload effectively and within organisational guidelines

About you

• Diploma, Bachelor's Degree in Community Services, Disability, Social Work, or a related field

• Experience as a NDIS Support Coordinator (essential)

• Strong knowledge of the NDIS and ability to navigate its processes

• Excellent organisational, communication, and stakeholder management skills

• Commitment to professional development and evidence-based practices

• Completion of the NDIS Worker Orientation Module

• Current Working with Vulnerable People Check (or equivalent)

• Current Driver's Licence and access to a reliable vehicle (if required)
