POSITION OVERVIEW

The Community Aged Care Nurse delivers nursing care directly in clients' homes and community settings, supporting older clients to remain safe, well, and independent. This is initially offered as a contract position, with the opportunity to transition to full-time employment in the future depending on caseload growth and operational needs. The role involves building strong relationships with clients and families, coordinating with other allied health professionals, and adapting care delivery to each client's home environment.

KEY RESPONSIBILITIES

• Conduct nursing assessments and health monitoring in clients' homes

• Develop and review individualised care plans in consultation with clients, families, and the broader care team

• Administer medications and clinical treatments as required

• Provide wound care, continence care, and other clinical nursing tasks as needed

• Identify and escalate changes in client health status promptly

• Maintain accurate, timely clinical documentation for each visit

• Support clients and families to understand and manage ongoing health needs

• Plan and manage a caseload of home visits across a designated area

QUALIFICATIONS & REGISTRATION

• Bachelor of Nursing (or equivalent)

• Current registration with AHPRA (Nursing and Midwifery Board of Australia)

• Working with Children Check

• NDIS Worker Screening Check

• Current Police Check

SKILLS & EXPERIENCE

• Experience in community, home-visiting, or aged care nursing

• Confidence working independently and managing a home-visit schedule

• Strong interpersonal skills for building rapport with older clients and families

• Good time-management skills to coordinate visits across a geographic area

• Current Australian driver's licence and access to a reliable vehicle
