The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including GP Clinics, Primary Health Care, Counselling, NDIS and community host various programs. At the heart of this role is walking side-by-side with Aboriginal and Torres Strait Islander community members as they explore the NDIS access pathway while supporting them to understand their options, build confidence, and achieve meaningful outcomes.

As a Community Connector - Access for the Ngala Kaaditj Community Impact Team, you’ll play an important role in helping Aboriginal and Torres Strait Islander people understand and access the National Disability Insurance Scheme (NDIS). You’ll be a trusted guide which includes raising awareness, providing culturally safe information, and empowering individuals, families, and carers to explore supports that improve quality of life.

In this role, no two days are the same. You’ll connect with community, collaborate closely with local organisations and service providers, and support individuals through every step of the NDIS application journey. Your work will help people feel supported, informed, and confident navigating a sometimes complex system.

If you’re community‑minded, culturally aware, and passionate about creating stronger pathways to disability supports—especially in remote areas—this role gives you the chance to make a meaningful impact every day.

Please note this role does require some travel to attend and coordinate events. This travel may include overnight stays in regional towns.

Selection Criteria:

• Demonstrated experience in advocating and supporting others to reach goals or outcomes
• Ability to support community members in reaching their goals-whether through encouragement, guidance, or helping them access services.
• Verbal and written communication
• Able to communicate clearly and confidently in both writing and conversation, using persuasive communication to influence, inform and build understanding.
• Demonstrated ability to build trust and rapport with individuals and groups to achieve positive outcomes for them
• Strong advocacy skills and confidence to advocate when others can't.
