Community Registered Nurse

Your Nursing Care PTY LTD is seeking a Community Registered Nurse to join our team in Hervey Bay, Queensland. This is a contract position that offers the flexibility to work within our aged care and NDIS community services. In this role, you will be at the heart of delivering compassionate, high-quality nursing care to elderly and disability clients in their homes and community settings. As an experienced community nurse, you will play a vital role in supporting our clients’ health, independence and wellbeing whilst working as part of a dedicated healthcare team committed to excellence in aged care.

Key responsibilities

• Provide direct nursing care to aged care, disability and private clients in community and home-based settings, including assessment, monitoring and intervention
• Conduct comprehensive health assessments and develop individualised care plans in consultation with clients, families and the multidisciplinary team
• Administer medications safely and accurately, maintaining detailed records and ensuring compliance with legislative requirements
• Monitor vital signs and health status, identifying changes and responding appropriately to client needs
• Perform wound care, catheter management and other clinical procedures as required
• Communicate effectively with clients, families, general practitioners and allied health professionals to ensure coordinated care delivery
• Maintain accurate and timely documentation of all client care provided
• Support clients in managing chronic conditions and promoting self-care and independence
• Contribute to a positive, respectful workplace culture that prioritises client dignity and person-centred care
• Adhere to all aged care and NDIS standards, policies, procedures and relevant legislation including aged care quality standards
• Ability to work one weekend per fortnight is must

What we’re looking for  

• Current registration with the Nursing and Midwifery Board of Australia (NMBA) as a Registered Nurse
• Demonstrated experience working in aged care, GP clinic, disability or community nursing and/or home-based healthcare settings
• Strong clinical knowledge and practical skills relevant to aged care or disability nursing
• Excellent communication and interpersonal skills with ability to build rapport with clients, families and multidisciplinary teams
• Ability to work independently and use clinical judgment whilst also collaborating effectively within a team environment
• Current First Aid Certificate and CPR certification
• Commitment to person-centred care, dignity and promoting client independence
• Knowledge of aged care quality standards and regulatory requirements
• Ability to manage workload priorities and work flexibly across different client locations within the Hervey Bay region
• Professional approach to confidentiality and data protection
