About the role
Are you a solution focussed individual with a passion for supporting people that are experiencing homelessness? We are on the hunt for a Service Navigator to join our team on a part-time basis (60 hours per fortnight) in Wagga Wagga, NSW (Wiradjuri Country).
This dynamic role focuses on building connections between individuals and local services, creating pathways for those experiencing homelessness, rough sleeping, or at risk of homelessness. The aim is to enhance access to primary care and support their well-being.

As a member of our team, you will play a vital role in supporting individuals to achieve positive outcomes and improve their quality of life. Key responsibilities include:
Deliver services within a person-centred, strengths-based framework that promotes dignity, respect and individual choice.
Manage and prioritise a diverse caseload, responding effectively to individual needs and urgent support requirements.
Develop and maintain collaborative partnerships with community organisations, government agencies and mainstream services to enhance client outcomes.
Support clients to build social connections, participate in community life and strengthen their support networks.
Advocate for and empower individuals to access the services and resources they need to achieve their goals.

To be successful in this role, you will have:
Relevant tertiary qualifications in Social Work, Human Services, Community Services or a related discipline.
A strong understanding of the challenges experienced by individuals and families affected by homelessness, including the impact of health, housing and social barriers.
Experience working effectively with Aboriginal and Torres Strait Islander peoples and communities, demonstrating cultural awareness and respect.
A commitment to person-centred, strengths-based and trauma-informed practice.
Proven ability to establish and maintain effective partnerships with internal and external stakeholders to achieve positive client outcomes.
Strong communication, relationship-building and case management skills.
