Occupational Therapist – Aged Care and NDIS

At Home Together is growing, and we are looking for an experienced, practical and compassionate Occupational Therapist to join our team.

This is a varied community-based role supporting both older Australians receiving Support at Home services and NDIS participants across Melbourne, Canberra and Queanbeyan.

The successful applicant can be based in either Melbourne or Canberra. You will primarily support clients in your local area, while also assisting clients in the other region via telehealth and occasional planned interstate travel where face-to-face assessments, equipment trials or home modification visits are required.

We are looking for someone who enjoys working directly with clients in their homes, completing meaningful assessments and helping people remain safe, independent and confident in their everyday lives.

The role will primarily focus on Occupational Therapy, with a smaller component of client coordination.

About the Role

Your responsibilities will include:

• Completing in-home Occupational Therapy assessments
• Functional and environmental assessments
• Assistive Technology assessments and recommendations
• Equipment prescription
• Coordinating and attending equipment trials with suppliers
• Completing home modification assessments
• Recommending modifications such as grab rails, ramps, bathroom modifications and accessibility solutions
• Preparing clear clinical reports and supporting documentation
• Preparing recommendations for aged care Assistive Technology and Home Modifications funding
• Completing NDIS-related OT assessments and reports within your scope of practice
• Liaising with equipment suppliers, builders, tradespeople and other allied health professionals
• Following equipment and home modification requests through from assessment to implementation
• Reviewing equipment suitability after delivery where required
• Completing telehealth assessments where clinically appropriate
• Supporting clients across Melbourne, Canberra and Queanbeyan
• Occasionally travelling interstate for planned assessment days
• Working closely with At Home Together’s care coordination, support coordination and operations teams
• Maintaining accurate clinical notes, reports and documentation
• Supporting a small number of client coordination matters where required

Client Coordination Component

This is primarily an Occupational Therapy position, not a care coordination role.

The majority of your workload will involve clinical OT work, with a smaller component involving client coordination and broader client support.

This may include:

• Reviewing client needs
• Coordinating services
• Following up equipment and home modification requests
• Communicating with clients, families and representatives
• Liaising with care partners, support coordinators and plan managers
• Assisting with care planning
• Supporting clients to navigate services
• Ensuring recommendations are followed through from assessment to implementation

We are particularly interested in someone who enjoys having ownership of the client journey rather than simply completing an assessment and handing over a report.

Location & Travel

The successful applicant can be based in either:

Melbourne, Victoria or Canberra, ACT

You will primarily support clients in your local region.

You may also support clients in the other region via telehealth, where clinically appropriate.

From time to time, you may be required to travel between Melbourne and Canberra for planned face-to-face assessments, equipment trials or home modification visits.

Where interstate travel is required, At Home Together will aim to coordinate multiple appointments together to make the trip efficient.

A company vehicle will be available for work-related client visits.

Approved interstate travel costs will also be covered by At Home Together.

About You

We are looking for an OT who is experienced, practical and confident working independently in the community.

Ideally, you will have experience in:

• Community Occupational Therapy
• Aged care
• NDIS
• Support at Home or Home Care Packages
• Assistive Technology
• Equipment prescription
• Equipment trials
• Home modifications
• Functional assessments
• Falls prevention
• Mobility and transfer assessments
• Working with older people
• Working with people living with disability
• Working with equipment suppliers and home modification providers
• Clinical report writing
• NDIS documentation
• Client or care coordination

Experience across both aged care and NDIS would be highly regarded.

Essential Requirements

You will need:

• Bachelor or Master’s degree in Occupational Therapy
• Current AHPRA registration
• Current Australian driver’s licence
• NDIS Worker Screening Check, or willingness to obtain one
• Working with Children Check where required
• Strong written communication and report-writing skills
• Excellent interpersonal skills
• Strong organisational and time-management skills
• Ability to work independently in the community
• Confidence liaising with clients, families, suppliers, trades and other professionals

The Person We Are Looking For

We want someone who can walk into a client’s home, understand what is happening in their day-to-day life and identify practical solutions.

You may be assessing someone who is struggling to safely shower, trialling a new walker with another client, helping organise a wheelchair trial, recommending bathroom modifications, speaking with a builder about access, or preparing clinical evidence to support an aged care or NDIS funding request.

We are looking for someone who can not only identify what a client needs, but also help make it happen.

You will be supported by our broader team, but you will also need to be comfortable making clinical decisions, managing your own workload and following client needs through to completion.
