About the Role:
We are seeking an energetic and enthusiastic Clinical Care Coordinator to join our team on a full-time basis!
You will be based at our St Ives Nursing Home, located at 97 Killeaton Street, St Ives NSW 2075.
We're looking for enthusiastic candidates to assist with overseeing the delivery of high-quality, person-centred care. This role provides clinical oversight, supports care teams, and ensures compliance with Strengthened Aged Care Standards in line with Thompson Health Care’s Philosophy of Care.
The successful candidate will:
Current AHPRA Registration (Essential)
Valid Police Check (Essential)
Tertiary qualifications in Nursing
Demonstrate strong leadership ability, as a Registered Nurse for a minimum of 3 years’ experience with a mix of acute clinical and aged care experience (desirable).
A good understanding of contemporary aged care, clinical practice and clinical leadership in a residential setting.
Maintaining accurate clinical documentation and ensuring confidentiality at all times
Delivering safe person-centred care that supports residents' health and wellbeing
Identifying, escalating, and responding to clinical concerns in a timely manner
Great communicate skills and confidence to guide and actively contribute to a team.
Strive for excellence in customer service and promote an environment of harmony and trust.
Demonstrated high level organisational and time management skills
Key Responsibilities
Provide direct clinical care, including assessments and evaluations as required
Coordinate care with multidisciplinary teams and external providers
Support Registered Nurses and Care Staff in delivering high-quality, person-centred care
Ensure clinical care aligns with organisational policies, procedures, and care philosophy
Maintain accurate and compliant clinical documentation, including relevant legislation and AN-ACC requirements
Monitor, investigate, and escalate clinical concerns and incidents appropriately
Build positive relationships with residents and families to support individual care needs
Ensure compliance with various regulatory requirements including WHS, infection control, fire regulation and accreditation requirements
